  • Telemedicine demonstration: Remotely operated biofeedback

    1.The ProComp 2 is a two channel physiological monitoring system that can be used to record physiological signals such as brainwaves (encephalography, or EEG), muscle activity (electromyography, or EMG), heart beat (electrocardiography, or ECG), finger pulse (blood volume pulse, or BVP), skin conductance (SC), temperature and respiration. This demonstration covers skin conductance and electromyography. The EMG sensor requires using electrodes, which are plastic or metal disks covered with a conductive substance. They snap onto the sensor and have an adhesive foam pad which is used to hold the electrodes against the client’s skin. The SC sensor has two short leads with electrodes on finger straps, which are fastened to two fingers of one hand. Usually, the index and ring finger. EMG measures the electrical activity of a muscle, in micro-volts. When the muscle contracts, small electrical potentials are picked up between the electrodes and this is shown on the screen as a line that moves up and down. Increasing muscle tension is shown as an upward signal. Skin conductance measures - indirectly - how sweaty the skin on the fingers is. The more stressed a person is, the more sweaty their hands are and the more conductive their skin is. This is also shown as an increasing signal value. For both physiological signals, the clinical target usually aims at lowering their value. The ProComp 2 is a two channel digitizer device, which samples the sensor signal 32 times per second and sends that data to the software via a USB interface. A fiber optic cable is used to send the data from the ProComp 2 to the USB interface because these cables are not susceptible to electrical interference and they help keep the client safe from risks of electrocution. (This is and FDA requirement for all such devices.)

    2.BioGraph Infiniti is a physiological monitoring and multimedia biofeedback software which is used to perform three main tasks: Recording, reviewing and reporting physiological data. As the sensors detect changes in the physiology, the recorded data is immediately converted to useful and understandable information and immediately shown on the screen. The data might be represented as a line moving across a graph or a bar moving up and down, like the red line on a thermometer. Multimedia feedback involves triggering various media actions from physiological conditions. For example, the theme from The Lab With Leo can start playing when the EMG signal decreases below a certain value, which would indicate that the client (Leo) has managed to relax his muscles below a threshold set on the graph. BioGraph provides clinicians with many multimedia options, like playing DVD movies, CD music or music files like Wav or MP3 songs.

    3.Biofeedback is a concept which has been around for many years. It involves measuring physiological signals and showing the change on some visual display, in real time. The client is asked to look at the display and make the line or the bar move up or down. The immediate feedback of success or failure helps the client learn to control a previously unconscious process. This is called self-regulation. Clinicians will generally assess the client’s baseline values and test their reaction to different stress situations. Then they will define target values and set thresholds on a display screen. The client gets rewarded by some kind of multimedia event (ex. playing music) when the threshold is reached. Clinicians use biofeedback for many applications, but the main ones are stress control and muscle rehabilitation.
    4.Telemedicine is a term used to describe any remote application of a clinical intervention. Among our customers, we have clinicians who use Internet based applications, such as Skype and GoToMeeting (desktop sharing by invitation), to give treatment in remote or tough to access places. Dr. Ray Folen, for example, works out of the Tripler Army Medical Center, in Hawaii, and is responsible for providing health services to active-duty service members of all branches of service across the entire Pacific Basin, including Japan, Korea and Guam. Instead of having the patients flown to Hawaii and housed for the duration of the treatment, Dr Folen has started using “telehealth” technology to provide treatment services to patients located thousands of miles away. The biofeedback system is set up at the remote site. The clinician controls the remote computer using commercially available web conferencing or remote PC control software, which also allows him to see what’s going on, on the screen. All that is required at the remote site is that a clinic assistant help the client get hooked up with the various sensors.

    5.Final point: The video game development world is becoming interested in biofeedback because it can provide new ways to expand the player experience. Imagine a game that would be able to sense the players stress – or boredom – levels and vary its challenges based on real-time physiological inputs. There are also interesting medical applications being developed, based on adapting virtual reality environments to the client’s physiological reactions, particularly in the field of phobic desensitization (fear of flying, fear of crowds and fear of public speaking). BioGraph offers a number of developer tools, including a dynamic link library of programming functions (SDK), that third-party developers can use to interface with our software and equipment.
